Substance Use Assessments

$450.00

Substance use assessments provide a clinical evaluation of substance use patterns, related concerns, functional impact, and treatment needs. A Clinical Substance Use Disorder (SUD) assessment typically includes a 60–90 minute evaluation that reviews substance use history, mental health factors, medical and social history, risk factors, and individual strengths.

Assessments may include:

  • Comprehensive clinical evaluation of substance use concerns

  • Standardized screening and assessment tools when appropriate

  • Diagnostic impressions and treatment recommendations

  • Written assessment reports when required

  • Recommendations for treatment, education, support services, or level of care

Assessments may be completed for personal insight, treatment planning, employers, schools, courts, probation requirements, or other organizations. Court-ordered and legal assessments are available and may include additional documentation requirements based on the purpose of the evaluation.

Additional services, including record review, communication with attorneys, probation officers, treatment providers, or other involved parties, as well as court testimony or depositions, are available when needed and are billed separately based on time and complexity.
